as a doctor a medication list is a must for an efficient office visit especially for those with 2 or more doctors.

the dose needes to be written along with the frequency of utilization. drug interactions are easier to pick up eg are you taking your thyroid pill with your centrum sikver for woman? if together the thyroid efficaccy goes way down. joe fallon md
